AndroGel and Low Testosterone

AndroGel is a prescription topical medication to treat hypogonadism (low T). It helps replace the natural testosterone your body is not producing enough of, bringing levels back to normal and improving symptoms of low testosterone. It is applied to the skin, allowing testosterone to be absorbed into the body.

AndroGel is not approved for use in individuals under 18 years old, as its safety and effectiveness in this age group have not been established.

Primary Hypogonadism

AndroGel is a prescription medication used as testosterone replacement therapy for men with low testosterone levels due to certain medical conditions. It is approved to treat primary hypogonadism, which can be congenital (present at birth) or acquired (developed later in life). This condition occurs when the testes cannot produce enough testosterone, often due to genetic issues, damage from infections, or treatments like chemotherapy.

Secondary Hypogonadism

AndroGel occurs when the brain's pituitary gland or hypothalamus fails to signal the testes to produce testosterone. This can result from injuries, tumors, or other medical conditions affecting these areas of the brain.

Late-Onset Hypogodanism

Late-onset hypogonadism is age-related low T where testosterone levels naturally decline, and for some, this can cause symptoms like fatigue, reduced muscle mass, low sex drive, and mood changes. AndroGel helps restore testosterone levels in these cases when both low levels and symptoms are confirmed by a doctor.

AndroGel is not approved for use in women or children and should only be used under a healthcare provider's guidance.

Application Instructions

AndroGel is applied differently depending on the concentration.

  • AndroGel 1%: Apply once daily in the morning to the shoulders, upper arms, and/or stomach area.
  • AndroGel 1.6%: Apply once daily in the morning but is limited to the shoulders and upper arms.

After application, always wash your hands thoroughly with soap and water. Allow the gel to dry completely before covering the application area with clothing, and keep it covered until the area has been washed or you have showered.

If there is a possibility of skin-to-skin contact with someone else, clean the application site with soap and water beforehand.

Note that if a child or woman touches the application site, the affected area should be washed immediately with soap and water.

Missed Dose

If you miss a dose of AndroGel, apply it as soon as you remember, unless it is almost time for your next scheduled dose. In that case, skip the missed dose and continue with your regular dosing schedule. Do not apply extra gel to make up for the missed dose, as this could increase the risk of side effects.

AndroGel should be applied at the same time each day, ideally in the morning, to maintain consistent testosterone levels.

Side Effects

Common Side Effects
  • Skin reactions at the application site, such as itching, redness, or rash
  • Acne
  • Increased red blood cell count (polycythemia)
  • Headache
  • Enlarged or tender breasts (gynecomastia)
  • Changes in sex drive (libido)
  • Elevated prostate-specific antigen (PSA) levels, which may lead to changes in prostate health

Before starting AndroGel, it is important to check hematocrit levels and assess them again 3 to 6 months after beginning treatment, followed by annual monitoring. If hematocrit becomes too high, treatment should be paused until levels return to a safe range. Elevated red blood cell counts can increase the risk of blood clots, which may lead to serious complications.

Serious Side Effects
  • Worsening prostate symptoms: If you already have an enlarged prostate, AndroGel could make symptoms such as urination changes more noticeable.
  • Increased prostate cancer risk: There may be a higher chance of developing prostate cancer while using AndroGel.
  • Blood clots:
    • Leg clots: Symptoms may include leg pain, swelling, or redness.
    • Lung clots: Signs may include chest pain or trouble breathing.
  • Heart attack or stroke risk: Using AndroGel could increase your likelihood of these events.
  • Lowered sperm count: High doses of AndroGel may reduce sperm production.
  • Swelling: This could occur in the ankles, feet, or other parts of the body, potentially worsening heart, kidney, or liver problems.
  • Breast changes: You might experience enlarged or painful breasts.
  • Sleep apnea: Breathing problems during sleep could occur or worsen.

If you experience any serious side effects, contact your healthcare provider immediately. If you have symptoms such as chest pain, difficulty breathing, or signs of a stroke, call 911 right away.

Warnings

  • Accidental exposure
    AndroGel can transfer to others through skin contact. Women and children who are exposed to testosterone gel may develop unwanted effects, such as body hair growth or other hormonal changes. Always wash the application area before close contact and ensure it remains covered.
  • Use in women and children
    AndroGel is not intended for women or children. Exposure in these groups can lead to significant hormonal disturbances or, in children, early signs of puberty.
  • Allergic reactions
    Some patients may experience serious allergic reactions to AndroGel, including swelling, rash, or difficulty breathing. These reactions require immediate medical attention.

Patients should use AndroGel exactly as prescribed, take precautions to prevent accidental exposure to others, and report any unexpected symptoms to their healthcare provider. Proper handling and storage are important to reduce risks.

Who Should Not Use AndroGel

Men with Breast Cancer or Prostate Cancer

The LidoPro patch is intended for adults aged 18 years and older and provides up to 12 hours of continuous relief when used as directed.

  • AndroGel is not suitable for men diagnosed with breast cancer or those who have been diagnosed with or are suspected of having prostate cancer.
  • Testosterone therapy can stimulate the growth of these hormone-sensitive cancers, potentially worsening the condition.
  • That is why regular monitoring is critical for men undergoing testosterone therapy to detect any early signs of prostate-related issues.
Pregnant Women
  • AndroGel is contraindicated for use in pregnant women due to the risk of harm to the developing fetus.
  • Testosterone exposure during pregnancy can lead to serious complications, including the development of male characteristics in female fetuses.
  • Women who are or may become pregnant should avoid contact with AndroGel and the skin areas where it has been applied to prevent accidental exposure.
