In the approval study for SOAANZ, they compared mean urine volume excreted up to 23 hours following administration of a single dose of SOAANZ 20 mg and DEMADEX (torsemide) 20 mg.  

SOAANZ was associated with a 32.4% lower urine volume excretion over 0-2 hours post-dose compared to DEMADEX. However, the mean urine volume excreted over 0-23 hours post-dose is similar between SOAANZ (4369 mL) and DEMADEX (4552 mL). 

Although both are torsemide molecules, DEMADEX is an immediate release tablet that delivers >80% of the drug into solution within 1-2 hours, whereas, SOAANZ is an extended-release tablet that delivers >80% of the drug into solution over 10-12 hours. 

The rationale for developing SOAANZ is to provide a gradual and sustained diuresis compared to DEMADEX and to alleviate the early abrupt or frequent urination that may be associated with the use of an immediate release formulation of torsemide.

SOAANZ is a novel formulation of torsemide (generic Demadex). SOAANZ is an extended-release version of torsemide which means that it was designed to release drug more slowly than generic version of torsemide allowing the medications to have prolonged action.

Soaanz can be taken at any time of the day. However, we tell our patients to try and take it at the same time every day so you don't forget to take it.

Although SOAANZ can be taken at any time of the day, it should be taken at roughly the same time every day so you don’t forget to take it.

However, if you do forget to take a dose, take it as soon as you remember unless it's almost time for your next dose. In this case, skip the forgotten dose and take the next one at the usual time. Do not take 2 doses together to make up for a missed dose.

It's crucial that your physician be aware of any additional medications you are taking because some of them might significantly raise your chance of developing serious issues.

SOAANZ is a CYP P450 2C9 (cytochrome P450, which is an important enzyme in your body that’s responsible for the metabolism of numerous drugs) inhibitor and inducer.

Therefore, medications that are CYP 2C9 inducers and/or inhibitors can affect SOAANZ and may need a dose adjustment, including the following medications:

• Amiodarone
• Fluconazole
• Miconazole
• Rifampin

Inform your physician of all prescription and over-the-counter (OTC) medications:

• Cholestyramine
  —However, if SOAANZ and cholestyramine are to be given at the same time, it's recommended to take Soaanz at least 1 hour before or 4-6 hours after cholestyramine.
• Desmopressin
• Probenecid
• Lithium
• Aminoglycoside antibiotics due to the increased risk of causing severe ear problems

In patients with non-anuric renal failure, high doses of torsemide (20 mg to 200 mg) caused marked increases in water and sodium excretion. In patients with non-anuric renal failure, severe enough to require hemodialysis, chronic treatment with up to 200 mg of daily torsemide has not been shown to change steady-state fluid retention. When patients in a study of acute renal failure received total daily doses of 520 mg to 1200 mg of torsemide, 19% experienced seizures. 96 patients were treated in this study; 6/32 treated with torsemide experienced seizures, 6/32 treated with comparably high doses of furosemide experienced seizures, and 1/32 treated with placebo experienced a seizure.

The safety and effectiveness of SOAANZ in pediatric patients has not been established. 

Administration of a loop diuretic to premature infants has been associated with the precipitation of nephrocalcinosis/nephrolithiasis. Nephrocalcinosis/nephrolithiasis has also been observed in children under 4 years of age with no history of prematurity who have been treated chronically with a loop diuretic. A loop diuretic, when administered during the first weeks of life, has also been reported to increase the risk of persistent patent ductus arteriosus.

Data regarding the usage of SOAANZ and the risk of severe birth abnormalities or miscarriages are unknown. However, you should still inform your doctor if you get pregnant or think you could be pregnant.

Your doctor will advise you on whether you should discontinue SOAANZ during pregnancy and if you should temporarily stop using it while nursing or breastfeeding. It is unclear whether SOAANZ enters breast milk. Prior to breastfeeding, speak with your doctor.

If you’re experiencing any of the serious side effects listed below, immediately call your doctor and stop taking this medication.

• Muscle cramps and/or weakness
• Extreme fatigue and/or dizziness
• Dry mouth
• Nausea
• Vomiting
• Increased heartbeat
• Numbness
• Tingling
• Pain, redness, and/or swelling
• Hearing problems, including ringing that occurs in your ear(s)

Certain potential adverse effects often do not require medical attention. Throughout therapy, these adverse effects could fade away as your body becomes used to this medication.

• Increased urination (not as much as other loop diuretics)
• Headache
• Dizziness and/or lightheadedness
  —When getting up from a sitting or laying posture, take your time to stand up so you don't feel lightheaded or dizzy.
• Your blood sugar levels may rise as a result of this medication.
  —Talk to your physician if you have diabetes and observe a change in the results of your blood or urine sugar testing.
• Dehydration or electrolyte imbalance, including low levels of sodium, potassium, magnesium, and/or calcium.
  —Dehydration can be increased by excessive perspiration, diarrhea, or vomiting.
  —Your blood potassium level may drop if you take SOAANZ. To stop potassium loss, your physician may advise you to increase the amount of potassium-rich foods in your diet, including bananas and orange juice.

SOAANZ comes as an extended-release (formulated to release the drug slowly in your body over a specific period of time) tablet and is available in a 40 mg tablet. Take it once a day by mouth, with or without food. For maximum effectiveness, take this medicine as prescribed and as directed.

The complete blood pressure-lowering impact typically takes 4–6 weeks, and occasionally up to 12 weeks, to become apparent. Do not discontinue taking SOAANZ without consulting your physician, even if you start feeling better.

SOAANZ is FDA-approved to treat the following medical conditions:

• Edema—(fluid retention or swelling that may occur in your lower limbs and/or stomach) that’s caused by congestive heart failure, liver disease, or kidney disease.
• High blood pressure (hypertension)—either by itself or in combination with other medications
  —Controlling your weight and being careful about what foods you consume, particularly those heavy in salt, may be used as additional treatments for high blood pressure.

It works by blocking sodium and chloride reabsorption in the ascending loop of Henle in your kidneys. This increases the excretion of sodium, potassium, chloride, magnesium, calcium, and water into urine, resulting in excessive urination as an adverse effect.

SOAANZ is used to decrease edema (excess fluid in your body) caused by heart failure, liver disease, and renal illness. This can minimize congestive symptoms such as breathing difficulties and arm, leg, and belly swelling.
